Superior HealthPlan Medicaid Coverage for Addiction Treatment

Promises DFW accepts Superior HealthPlan Medicaid for primary substance use treatment for adults 18+. Our campus is at 400 Highland Dr, Lewisville, TX 75067. Admissions checks your specific benefits and active eligibility, along with the prior authorization needed for the care recommended to you.

Superior is the largest Medicaid managed care plan in Texas. Its Medicaid coverage is distinct from Ambetter, a commercial insurance product. If you recognize both names, start with the product shown on your current card. That helps admissions review the correct benefits.

Superior Medicaid and Ambetter are different products

A familiar insurer name can make it easy to land on the wrong coverage page. Superior HealthPlan Medicaid is for Medicaid members. Ambetter is commercial insurance, with a separate benefits review. Promises DFW accepts both, but a coverage answer for one product does not establish what the other will pay for.

If your card identifies Ambetter, use our Ambetter from Superior HealthPlan insurance page when you discuss your policy with admissions. For a Superior Medicaid card, ask for a Medicaid addiction treatment benefits check. If you are unsure which product you have, read the name and member information from the card during the call.

What Superior Medicaid can be used for here

At Promises DFW, Superior HealthPlan Medicaid is accepted for addiction treatment only. It does not cover primary mental health care at this facility. Someone seeking primary mental health treatment should contact admissions about commercial insurance or other ways to access care.

Medical detox and residential addiction treatment are available in Lewisville. Whether your Superior Medicaid plan will help pay for either service depends on active eligibility, the clinical review and the plan’s prior authorization. PHP in Lewisville and virtual IOP for Texas residents may be options, subject to the same individual review. A service being available at the facility does not establish coverage under your policy.

Before you request a benefits check

Find your current insurance card or benefits information. Have these details ready:

  • The full product name, including whether the card says Medicaid or Ambetter.
  • Your member ID and the information needed to locate your enrollment.
  • Your current contact details so admissions can explain the results.
  • Any notice about a renewal, coverage change or pending eligibility decision.

You can call even if you are missing a document. Tell admissions what information you have and ask what else is needed. A relative helping you can write down questions about coverage, but the team will explain what it needs to verify the member’s benefits. Avoid treating an older card as confirmation that coverage is still active.

How Superior prior authorization affects admission

The benefits check identifies the policy and what it requires. Prior authorization is the plan’s review of the requested treatment. Admissions checks which approval is needed for the recommended level of care and what information the plan needs. Superior makes the authorization decision; the facility cannot promise approval.

Ask which service is being requested and whether the request is still under review. Also ask whether any covered dates or further reviews affect the proposed treatment. Authorization for an initial service should not be treated as approval for every later stage of care.

Before making the trip to Lewisville, confirm the admission arrangements with the team. A completed benefits check alone does not tell you when treatment can start. Admissions can explain which questions have been answered and which depend on the plan’s response.

Adult eligibility for Texas Medicaid

Texas has not expanded Medicaid. Some pregnant adults, some parents and caretaker relatives with very low income, people 65+ or with disabilities, and former foster care youth up to age 26 may qualify. Being an adult who needs addiction care does not itself establish eligibility.
