Finishing a treatment program is an accomplishment worth recognizing. It is also a transition. The structure, support and daily therapy of treatment give way to the demands of work, relationships and everyday stress, and that shift is when many people feel especially vulnerable. Aftercare is the plan that helps carry your progress forward.

At Promises Dallas-Fort Worth, every client leaves with an aftercare plan. That plan is built with you before you go, and it reflects your goals, your needs and the resources available where you live.

Why Aftercare Matters

The National Institute on Drug Abuse describes addiction as a chronic, treatable condition. Like other chronic conditions, it often calls for ongoing care rather than a single episode of treatment. NIDA also notes that a return to use does not mean treatment has failed. It usually means the plan needs to be adjusted or support needs to increase.

Mental health conditions such as depression, anxiety, bipolar disorder and PTSD also tend to need continued attention after an intensive program ends. Aftercare helps you keep practicing the skills you learned, stay connected with care providers and respond early if symptoms return.

What an Aftercare Plan Can Include

No two aftercare plans look the same. Depending on your needs, yours may include:

  • Step-down care: Moving from residential treatment into our in-person partial hospitalization program in Lewisville, or into our virtual intensive outpatient program for adults anywhere in Texas.
  • Outpatient referrals: Referrals to therapists, psychiatric providers and other clinicians for ongoing therapy and medication management closer to home.
  • Support groups: Suggestions for community mutual-support meetings, including 12-step and non-12-step options, that match your preferences.
  • Relapse prevention planning: A written plan that identifies your triggers, warning signs and the specific coping skills and contacts you will use.
  • Family involvement: Guidance for loved ones on how to support your recovery while taking care of themselves.
  • Medication continuity: If you take medication, a plan for continuing it with an outside provider.

How Aftercare Planning Works

Aftercare planning starts before discharge. During treatment, your team learns about your life outside the program: where you live, whether you are working, who supports you and what challenges you are likely to face. As treatment continues, you and your team shape a plan around that picture.

Planning is collaborative, and you have a say in which options feel realistic. If you prefer in-person care near home, your team can look for local referrals. If your schedule or location makes travel hard, virtual IOP may be a better next step. If you have been in residential mental health treatment, your plan may emphasize psychiatric follow-up and therapy. If you have been in addiction treatment, it may focus more on relapse prevention and recovery support.

Building a Relapse Prevention Plan

A relapse prevention plan is a practical, written tool you can return to when things get hard. It often includes:

  • People, places and situations that raise your risk
  • Early warning signs, such as skipping appointments, isolating, losing sleep or romanticizing past use
  • Coping skills from CBT, DBT and mindfulness practice that work for you
  • A list of people you can call, including outpatient providers and supportive friends or family
  • Steps to take after a slip, so a single lapse does not turn into a full return to use

Having this plan in writing matters, because stress can make it hard to think clearly in the moment. Keeping a copy on your phone makes it easy to find when you need it.

The Role of Family in Aftercare

Recovery affects the whole family, and loved ones often want to help but are not sure how. Family therapy during treatment can lay the groundwork by improving communication and setting healthy boundaries. Knowing When You Need More Support

Part of a good aftercare plan is recognizing when to ask for more help. Recovery can shift over time, and needing a higher level of care for a while is not a failure. It is a sign that you are paying attention to your health. Consider reaching out to your providers or our admissions team if you notice:

  • Cravings that are stronger or more frequent than before
  • A return of symptoms such as low mood, panic, racing thoughts or trouble sleeping
  • Pulling away from the people and routines that support your recovery
  • Stopping medication or missing appointments without talking to your provider

Depending on what is happening, returning to PHP or virtual IOP for a period of time may help you regain stability before things become harder to manage.

Does every client get an aftercare plan?

Yes. Every client at Promises DFW leaves with an aftercare plan tailored to their needs, whether their care included detox, residential treatment, PHP, virtual IOP or a combination.

What if I live outside Dallas-Fort Worth?

Your plan can include outpatient referrals near your home. Adults anywhere in Texas may also be eligible for our virtual intensive outpatient program.

What should I do if I return to use after treatment?

Reach out for help as soon as you can. Contact your outpatient provider, follow the steps in your relapse prevention plan or call our admissions team. If you used opioids before treatment, remember that tolerance drops after a period of not using, which raises the risk of overdose. Call 911 in an emergency, and call or text 988 if you are in a mental health crisis.

Is aftercare covered by insurance?

Coverage for step-down programs and outpatient care depends on your plan, medical necessity and prior authorization. Our team offers free, confidential insurance verification.
